I have tried for about 30 minutes to find a source episode number (and, more importantly, script) ... I believe it's just a sign of the times. Back then, your wife was your second head. Or husband. Whatever. They were your second head. It was immediately understood and interpreted that way. These days, it's not commonplace unless context is specifically pointed out.

I do find it suspicious that the exact quotes from the clip don't return any results on google, which makes me think it's a chop-job. Most scripts can be found online and results show up for 10-word-long quotes. But I dunno.

Another possibility is the costume they were doing had two heads. There's just no way to know without the original script or a killer memory.
